Association membership form
A member register that lives in one place instead of one person's inbox. It collects the details you actually need, asks whether someone wants to help out, and records consent to your privacy policy with the registration it belongs to.

This is the actual form the template creates, shown as your respondents would see it.

Features it uses
Consent recorded with the answers
The consent field carries your own text, links included, and is stored as part of the submission.
Ready for spreadsheet sync
Switch on Google Sheets or Excel Online sync and the member list keeps itself current. Needs Pro or Business.
Shared with your committee
Invite the rest of the committee into the workspace so the register survives a change of role.
Retention you set
Submissions are deleted automatically after the period you choose, on every plan.
Making it yours
- 1
Copy it into your workspace
It arrives unpublished, so you can rework it before anyone sees it.
- 2
Rewrite the consent text
Replace the placeholder wording with your own privacy statement and link to your policy.
- 3
Set the retention period
Decide in the workspace settings how long member registrations are kept.
- 4
Publish and invite your members
Share the link in your newsletter or on your site.
Common questions
- Can I collect the membership fee on this form?
- Yes. Enable payments on the form and attach an amount to the membership type, the way the sports club template does. Payments need Pro or Business.
- Can several people manage the register?
- Yes. Invite them into the workspace. Free has one seat, Pro five and Business twenty.
- How do I get the list into a spreadsheet?
- Export to CSV on any plan, or turn on live sync to Google Sheets or Excel Online on Pro and Business.
