Per-option pricing
Each membership type and each extra carries its own amount, and the total adds up as the member fills in the form. Payments need Pro or Business.
File upload
The member photo is stored on a private disk and downloadable only from your workspace. File uploads need Pro or Business.
Consent fields
Photo consent and club-rules consent are recorded with the submission, each with your own wording beside the checkbox.
Confirmation emails
The member receives their answers by email and the registrar gets a notification. Available on every plan.

The template arrives as an unpublished draft, so nothing is live until you say so.

Set your own fees
Change the membership names and the amount attached to each, and remove any extras you do not sell.

Connect Mollie
Add your own Mollie API key in the workspace payment settings so fees are paid directly to the club.

Send it to your members. Payment status is recorded per registration.
Common questions
- Can I add a limit per team or age group?
- Yes. Turn on limits for the membership field and give each option a number of places, with a counter showing how many are left.
- Can I use it without collecting payment?
- Yes. Switch payments off on the form and the priced options simply become ordinary choices.
- What if I do not want to collect a photo?
- Delete the photo field and the photo consent that goes with it. Nothing else depends on them.
