School trip consent form
The paper slip, without the schoolbag. Guardians confirm permission, leave an emergency number and sign on screen, and the medical and dietary questions only appear for the pupils who are actually coming.

Features it uses
Conditional questions
The dietary, medical and certificate fields appear only when permission is given, so nobody answers questions that do not apply.
Signature field
The guardian signs on a phone or tablet, and the signature is stored with the submission.
Private file upload
A medical certificate is stored on a private disk, downloadable only from your workspace. Needs Pro or Business.
Retention that matches the trip
Set a short retention period so medical details do not survive into the next school year.

One link by email or your school platform, and one list you can check before departure.
