Feedback survey form

Short enough that people finish it. One rating, one recommendation score and two open questions, with the email address asked only of the people who want to hear back, so the rest can answer anonymously.

This is the actual form the template creates, shown as your respondents would see it.

How did we do?

A few short questions. It takes about a minute.

Replace this paragraph with what you are asking about and what you will do with the answers. People answer more honestly when they know why you are asking.

0 is not at all, 10 is definitely.

Conditional

Only used to reply to this feedback.

Who it is for

Anyone who wants a number they can compare next time, not just a feeling.

  • Event organisers asking attendees afterwards
  • Schools and courses collecting evaluations
  • Businesses measuring satisfaction after delivery

What is in the form

Eight items, most of them one tap.

  • An introduction paragraph explaining why you are asking
  • An overall star rating
  • A nought-to-ten recommendation score
  • Which parts worked, as a tick list
  • What went well and what could be better, as open questions
  • Whether they want a reply, with the email address shown only then

Features it uses

Ratings and sliders

A star rating and a nought-to-ten slider give you numbers you can average, rather than adjectives you cannot.

Anonymous unless they say otherwise

The email field appears only for people who ask to be contacted, so answering costs nothing.

Ready to chart

Export to CSV on any plan, or sync live to a spreadsheet on Pro and Business.

Making it yours

  1. 1

    Copy it into your workspace

    The rating scales come configured.

  2. 2

Name the thing you are asking about

Put the event, course or delivery in the opening paragraph and in the questions.

  3. 3

Send it while it is fresh

Response rates fall off quickly. The day after beats the week after.

Common questions

Is it really anonymous?
No name or email is asked unless someone wants a reply. Submissions do record an IP address for spam protection, which you can see in the workspace.
Can I see the averages without exporting?
Yes, on the Insights tab of the form. Insights need Pro or Business.

Ask while they still remember

Copy the template, name what you are asking about, and send it.
