Common questions
- Can I ask for a national registry number?
- You can add a field, but only collect it if you have a legal basis. The template leaves it out on purpose.
- Can two guardians each fill in their own part?
- Not in one submission. The template takes a second guardian as a name field; run a second form if both need to sign separately.
